What is tesamorelin?

Tesamorelin is a synthetic analogue of growth hormone-releasing hormone (GHRH). It contains the full 44-amino-acid sequence of human GHRH with a trans-3-hexenoyl group attached to the N-terminus. The molecular weight is approximately 5.1 kDa.

Research background

Tesamorelin was developed by the Canadian company Theratechnologies. The N-terminal modification was designed to reduce degradation by the enzyme DPP-4, which rapidly inactivates native GHRH. A regulated pharmaceutical preparation is approved in some countries for a specific indication.

Mechanism and scientific context

Tesamorelin activates the GHRH receptor on pituitary somatotrophs, stimulating GH release and consequently IGF-1 signalling. Unlike secretagogues such as ipamorelin, it acts via the physiological GHRH pathway and preserves the pulsatile release pattern.
